How To Get Started

Step 1: Complete the Online Initial Intake Form.

Complete this easy ONLINE INTAKE FORM with general information about your autism assessment and services needs. Someone from our staff will reach out to you.  

Step 2: Complete Your Intake Packet.

You will receive an email with directions on how to complete additional intake forms required for further evaluation. Please gather the following items:

1. Insurance documentation

2. IEP (Individualized Education Plan) or 504 plan if you have one.

3. Copy of any school evaluations done to determine school services or IEP.

4. Any other previous clinical evaluations.

Once complete, our intake team will begin the next steps.

Step 3: Clinical Review Call

Within a couple of days of receiving the completed intake packet and records, our intake team will reach out to confirm if AAC is an appropriate fit for your family. We will answer questions you may have regarding the assessment process.
